Dr Chan Po Fun is a Consultant Cardiologist with Cardiac Care Partners, and a Visiting Consultant with National University Health System. She has subspeciality interests in Heart Failure, Cardiac Imaging, and Women's Heart Health.

She graduated from the Yong Loo Lin School of Medicine, National University of Singapore. She furthered her training in Internal Medicine, and then Cardiology at the National University Hospital, and was accredited as a Specialist in Cardiology by the Ministry of Health. She received her testamur from The National Board of Echocardiography (America), and Certification in Heart Failure from the Heart Failure Association (European Society of Cardiology).

Prior to private practice, she was Clinical Lead for Heart Failure Services at Ng Teng Fong General Hospital, where she established a multi-disciplinary Heart Failure Team that was awarded a top Quality Improvement Project award. She also represented the hospital on the Ministry of Health (Singapore) Clinician Advisory Workgroup for Heart Failure, and was also the Champion for the hospital's Congestive Heart Failure Value Driven Outcomes project. 

 

Dr Chan was also Director of Cardiac Diagnostic Laboratory at Ng Teng Fong General Hospital. She has an interest in 3D echocardiography and strain imaging. She is also accredited in performing transesophageal and stress echocardiography.

Her friendly demeanour and sound knowledge has gained her respect and trust from many of her patients and colleagues; Dr Chan was awarded the Service Ambassador Award on several occasions.

​

Dr Chan has also mentored medical students, junior doctors, nurses, and cardiac technicians, and was a Faculty Member of the Cardiology Senior Residency Programme at the National University Heart Centre.

 

In recognition of her professional contributions to the field of cardiology, Dr Chan has been elected Fellow of the Academy of Medicine, Singapore.
